INCOMING 6TH GRADE

In compliance with the Department of Education Board Policy 337 and SOP 1200-020 (Health Requirements for Students) all incoming 6th graders and 9th graders, as well as students who are new to GDOE public schools, are required to update their Physical Examination and PPD Skin Test before entering any GDOE school. If the student is known to have Positive/Latent TB result, a clearance from Department of Public Health and Social Services TB Program in Dededo is also required.
When registering your child, we will need a copy of the completed Physical Form and your child's updated shot record.
Below is the GDOE PATH (Providing Access To Homes) Application for a Mi-Fi device. ONE application per household. Households with multiple children must apply at their eldest child’s school. Submit completed forms to the main office. Email preferred.
Parents/Guardians may check out the electronic devices at their child’s school. Families with multiple students at different schools must apply for/obtain a device from each of their children's respective school.
Please submit one application per student. This application is for students attending F.B.L.G. Middle School. By completing this application, the parent/guardian/student agree that the device is strictly for educational purposes. Application must be complete before submitting it.

This Learning Management System (LMS) allows you to quickly check on your students' reported progress in their classes, attendance, as well as any other notes their teachers may have about them. There is also a mobile app available.
